Dr. Berkovich is committed to providing you with the best possible treatment planning and dental care available. Dental treatment can be an excellent investment, not only for an individual’s medical well being, but psychologically as well. It is our commitment to you to keep financial considerations from becoming an obstacle to obtaining this important health service. Being sensitive to the fact that people have different needs in fulfilling their financial obligations, we are providing the following payment options:

• Cash, personal check or money order
• Credit cards: We accept Visa, MasterCard, American Express, and Discover
• Flexible monthly payment options (dental fee plan)
• No initial payment
• Available for treatment fees between $300 and $25,000
• Payment plans up to 60 months with low monthly payments
• Prepayments can be made anytime without penalty

• Insurance Assignment: Our office works with many dental insurance companies excluding HMOs. As a courtesy to our patients we can electronically file your insurance claims having them send payment directly to us. You, the patient, will be required to pay an estimated amount in which the insurance does not cover.

Please note that not all services are a covered benefit in all contracts. We must emphasize that as dental care providers, our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered.
